Narrow fix: completed checkpoints no longer suppress independent semantic assessment against the original task. Only a valid continue plan replaces completed. Waiting/blocked, cancellation, pause, deadlines, identity/depth guards and durable queueing remain intact. No supervisor/bootstrap/review changes.

Verification: 72/72 focused tests (completion-check, harness, task-recovery, result-orchestration, result-tool-hook); npm run typecheck; npm run build; git diff --check. Core review passed without findings: e240fb983ff691bd8b52d1adc213586f7ddb68e07384b2d455f18d9aa2707db6. GitOps activation separate; no restart/sync performed.
