Stacked on PR48. Adds plugin_task_continuation structured continue/waiting/blocked/completed plan, at most one checkpointed follow-up, 20-step chain bound, failed/cancelled/API exclusions, stale replay plan reset. Removes automatic five-minute model heartbeat initialization; explicit heartbeat schedules retained. Nightly controller uses local-time Cron start/end, committed hook continuation and state-only recovery Cron. Deferred Cron intent persists across busy sessions/restart. Review findings deferred-start, stale replay and unreserved nightly follow-up confirmed/fixed. 310 full tests before final nightly exclusion, then 45 targeted plus typecheck/build passed. Final review 1be7baff6f255c0cf684fe6dceec9a3b4db32142bc872834b9cf866382e7022d no findings. No rollout claim; evaluator/adoption safety limits from PR48 remain.
